If you are a medical professional and you disclose Personal Information including health information about a patient, we will use the information you submit to provide the services you request and in accordance with this Privacy Policy. As a medical professional, solely you have the obligation and responsibility to obtain any patient consents or authorizations for such disclosures that may be required under federal and state law.  By disclosing Personal Information regarding another individual to SalutarisMD, you represent that any such required consents and authorizations have been obtained.
